Living with a long-term condition like diabetes can be challenging, but you don’t have to manage it alone. At Health New Lynn, we offer a free, supportive, community-based programme that helps you build confidence, improve your health, and connect with others who understand your journey.
About the programme
Our Self-Management Education & Shared Medical Appointments (SMA) programme provides education and personalised care in a relaxed group setting. You’ll meet with a team of health professionals and up to 20 other patients who share a similar condition – working together to learn, grow, and manage your health more effectively. The sessions will feature a team of healthcare professionals, including nurses, a clinical pharmacist, a health coach, a health improvement practitioner, and a care coordinator.
You’ll receive:
- One-on-one time with healthcare professionals
- Education resources to take home after the session
- Lab results discussed on the spot
- Optional physical exams in a private breakout room
- Support from others who “get it”.
During our sessions, you’ll gain valuable skills and knowledge, including:
- Setting personal goals and solving everyday challenges
- Understanding your condition and what it means for you
- Keeping track of your health numbers
- Managing food, activity, stress, medication & sleep
- Navigating the healthcare system
- Preventing complications (e.g., eye, foot, kidney care)
- Staying safe – what to do if you’re unwell or your sugar levels drop.
When and where will this be?
- Wednesday 7 May, Thursday 22 May, and Wednesday 11 June, 2025
- 10am – 12pm (60-minute sessions)
- Free of charge
- Held in a welcoming, safe, and comfortable space at Health New Lynn.
Who can join?
Anyone enrolled with Health New Lynn within metro Auckland, aged 15 and over, is welcome to attend. We encourage Māori, Pacific, CSC holders, or those living in high-needs areas (Quintile 5) to take part. This programme is specifically for people with diabetes, and their whānau/support people.
